Respite Grant Program
Terms and Conditions of Acceptance
This document outlines the terms and conditions associated with accepting a grant from the Greater Lansing Care Foundation (GLCF) under its Respite Program. By accepting a grant, the recipient (“Grantee”) agrees to the following:
1. Purpose of Grant
The grant funds must be used solely for respite care services for caregivers of individuals with dementia or related conditions. These services may include:
In-home professional respite care
Adult day programs
Short-term out-of-home respite (e.g., residential stays)
Related expenses that facilitate caregiver rest and recovery
2. Grant Amount
Each grant provides up to $1,000 per calendar year per eligible household. Funds are not guaranteed and are contingent upon availability.
3. Use of Funds
The Grantee agrees:
To use funds exclusively for respite-related expenses as stated in the application
To retain all receipts and records of payment for at least 12 months
To return any unused or misused funds to GLCF upon request
4. Eligibility Confirmation
The Grantee affirms that:
They are a primary, unpaid caregiver for a loved one living with memory loss or a related cognitive condition
They have submitted accurate and truthful information on the application
They meet all eligibility requirements set forth by the GLCF Respite Program
5. Documentation and Follow-Up
GLCF reserves the right to request follow-up information or brief evaluations to assess the impact of the grant. The Grantee may be asked to participate in a short phone call or complete a feedback form.
6. Non-Transferability
Grant awards are not transferable and may only be used by the individual(s) identified in the grant application.
7. Publicity and Consent
The Grantee may be asked for voluntary consent to share their story (non-identifying unless permission is granted) for GLCF promotional or reporting purposes. Participation is entirely optional.
8. Revocation of Grant
GLCF reserves the right to revoke the grant and request reimbursement if:
Funds are used fraudulently or for purposes not approved
The application is found to contain false information
The Grantee does not comply with the terms listed herein
9. Indemnification
The Grantee agrees to indemnify and hold harmless the Greater Lansing Care Foundation from any and all claims, liabilities, or expenses arising from the use of the grant funds.
